Teoretická část bakalářské práce se zabývá definicí pojmu chronická rána, popisuje fáze hojení, faktory ovlivňující hojení ran a zaměřuje se na oblasti, které je třeba hodnotit v rámci ošetřovatelské péče o chronikcou ránu. Dále teoretická část stručně popisuje metody léčby chronických ran.
V praktické části jsou zpracovány dvě kazuistiky zaměřené na hodnocení chronické rány. Jsou zde určeny ošetřovatelské problémy a následně vypracován plán péče o pacienta. V diskuzi porovnávám ošetřovatelskou péči prostřednictvím agentury domácí péče a ošetřovatelskou péči o nehojící se ránu v nemocničním prostředí. Dále zde srovnávám ošetřovatelské problémy pacientů v obou kazuistikách. Celá práce je doplněna o "Záznam hodnocení nehojící se rány", který je obsažen v přílohách práce.
Annotation in English
The theoretical part of the thesis deals with the definition of chronic wound, describes the healing phase, factors affecting wound healing and focuses on the areas that need to be assessed in the context of nursing care of chronic wounds.
In the practical part are presented two case reports focused on the assessment of chronic wound. There are nursing problems identified and subsequently be drawn up a plan of care of the patient. In the discussion I compare the nursing care through home care agencies and nursing care for non-healing wound in a hospital environment. I also compare the nursing problems of patients in both care reports. All work is sompleted by "Document rating non-healing wound" which is contaied in the Annexes to work.
